some tweaks to make dist process:
- don't include .d files (this fixes hung make bug reports)
- make dist now fails if any build steps fail (such as making docs)
- distclean no longer removes pregenerated docs in release tree
- make dist now relies on configure and distclean being run in constructed
  (tmp) directory rather than local directory.  This allows it to produce
  same results whether make dist is invoked from in tree or out of tree build.

a 'make dist' start.  the script's in maint/make-dist.sh.  it might almost
work.  if not, please fix it.
